HACC multi service outlet providing a large range of services.
HOME & COMMUNITY CARE MULTI SERVICE OUTLET
The aim of the Service is to assist people to remain in their own home safely and to avoid inappropriate or early admittance to nursing home or hostel accommodation.
ALL CLIENTS will need to be assessed to determine eligibility for HACC service.
HOME MAINTENANCE & MODIFICATION
Examples of Modification Work;
*Replacing bath with shower, Hand held showers, grabrails and handrails, ramps and modifying steps
Examples of Home Maintenance;
*Cleaning gutters, yard maintenance, general house repairs including plumbing and electrical.
FOOD SERVICES
Good food helps you to maintain a healthy life. Those who have difficulty shopping or preparing their meals now have a range of meal options to choose from. Diabetic and other special food needs are catered for. You may wish to utilise.
Meals on Wheels; Delivered Hot Lunchtime Meal or Frozen Meals.
Community Restaurant; Usually held the first sunday of the Month.
Shopping Service; To assist you with your grocery shopping.
NEIGHBOURAID/SOCIAL SUPPORT
Friendly home visiting, social groups
DOMESTIC ASSISTANCE & RESPITE CARE
We have limited funding for these two services which can provide house cleaning or care for someone while their usual carer has a break from caring.
COMMUNITY TRANSPORT
The Community Bus and Car are available for both local transport for shopping and appointments or out of town transport.
The bus is also available for hire to community groups, please contact reception staff for more details.
